Liability Waiver & Medical Authorization

By registering my child(ren) for Emerald Crossing VBS 2026 at House of Grace Church, I acknowledge that I have read, understand and agree to the following: (a) Participation & Assumption of Risk I give permission for my child(ren) to participate in all activities associated with Emerald Crossing VBS 2026, including but not limited to worship, games, crafts, outdoor activities, and group events. I understand that participation in these activities involves inherent risks, including minor injuries, falls, or allergic reactions. I voluntarily assume all risks associated with my child's participation. (b) Liability Release To the fullest extent permitted by law, I release and hold harmless House of Grace, its pastors, staff, volunteers, and representatives from any and all claims, demands, or liability arising from my child's participation in Emerald Crossing VBS 2026, except in cases of gross negligence or willful misconduct. (c) Medical Authorization In the event of an emergency and if I cannot be reached, I authorize House of Grace and its representatives to obtain emergency medical treatment for my child as deemed necessary. I understand that I am financially responsible for any medical expenses incurred. I acknowledge that I am responsible for providing accurate information regarding my child's medical conditions, allergies, or special needs during the registration process. (d) Acknowledgement By checking the box below, I certify that I am the parent or legal guardian of the registered child(ren) and that I have read, understand, and agree to the terms of this Liability Waiver & Medical Authorization.
