By registering to serve as a volunteer for Emerald Crossing VBS 2026 at House of Grace, I acknowledge that I have read, understood and agree to the following:
(a) Voluntary Participation
I understand that my service as a volunteer is entirely voluntary. My responsibilities may include, but are not limited to, assisting with children, games, crafts, worship, snacks, setup, cleanup, supervision, and other VBS-related activities.
I acknowledge that volunteering may involve physical activity and interaction with children and groups, which carries inherent risks.
(b) Assumption of Risk
I knowingly and voluntarily assume all risks associated with my participation as a volunteer, including but not limited to minor injuries, illness, accidents, or other unforeseen incidents.
(c) Liability Release
To the fullest extent permitted by law, I release and hold harmless, House of Grace, its pastors, staff, volunteers, and representatives from any and all claims, demands, damages or liability arising from or related to my participation as a volunteer for Emerald Crossing VBS 2026, except in cases of gross negligence or willful misconduct.
(d) Medical Authorization
In the event of an emergency and if I am unable to make decisions for myself, I authorize House of Grace and its representatives to obtain emergency medical treatment on my behalf. I understand that I am financially responsible for any medical expenses incurred.
(e) Background Check Acknowledgement
I understand and acknowledge that all volunteers serving at Emerald Crossing VBS 2026 at House of Grace are required to have a current background check on file as part of the church's commitment to providing a safe and secure environment.
By registering as a volunteer, I consent to the completion of a background check if one is not already on file or if my existing background check is not current, in accordance with House of Grace policies and applicable laws.
I understand that any background check information will be handled confidentially and reviewed only by authorized church leadership for the purpose of determining volunteer eligibility. Background check results will not be shared beyond those with a legitimate need to know.
I acknowledge that my eligibility to serve is contingent upon satisfactory background check results and that refusal to consent will result in disqualification from volunteering.
(f) Youth Volunteer Acknowledgment
If I am under the age of 18, I understand that I am considered a youth volunteer and may be subject to additional supervision, role limitations, or requirements as determined by House of Grace. I agree to follow all instructions, safety, guidelines, and expectations set forth by VBS leadership. Parent/Guardian consent may be required separately.
(g) Code of Conduct Acknowledgment
I agree to conduct myself in a manner consistent with the values, beliefs, and expectations of House of Grace. I understand that I am expected to follow all VBS policies, safety procedures, and instructions provided by church leadership, and that failure to do so may result in dismissal from volunteering.
Photos and/or videos will be taken during Emerald Crossing VBS 2026 for use by House of Grace, including but not limited to church communications, website, social media, and promotional materials.
By Volunteering, I acknowledge that my image or likeness may be captured during VBS activities. If I do not wish for my image to be used, I understand that House of Grace will make reasonable efforts to honor this request; however, due to the group nature of VBS activities, complete exclusions from all photos or videos cannot be guaranteed.
